You will receive a jury questionnaire in your jury packet.

 

The questionnaire must be completed entirely, both pages, before returning to the court at:

Please return your completed questionnaire within the time allotted in your jury letter.

Remember to include your contact information on the first page of the questionnaire should the court need to reach you.

 

Also, please do not fax your questionnaire as we do need the original.

Your questionnaire will be reviewed by the court and the trial attorneys and will be destroyed after your service is complete.
