If you are unable to serve as a juror due to a physical, mental or financial disability, you must notify the Court in writing as to the nature of your disability and secure an excuse from reporting. Please enclose your request with your completed questionnaire when it is returned to the Court.  And please note that no one can excuse you from jury service except for the Judge who is presiding over the case for which you have been summonsed.  In determining whether to grant your request for an excuse, the Judge will follow the guidance set forth in Ohio Revised Code 2313.14.

 

No requests will be considered without the completed questionnaire.
